| Brief Detail: | Geriatric Nurse Practitioner/Clinical Supervisors Needed! Openings in NYC, Westchester County, Staten Island and the 5 boroughs. This Nurse Practitioner will act as a consultant on general medical issues for Care Managers and Primary Care Physicians. This will include providing and coordinating medical care to patients in nursing homes in conjunction with PCPs and Medical Directors. You will also make home visits to complex patients and advise Care Managers on developing their care plans.This position requires NY NP licensure, registration and DEA certificate plus at least one year of NP experience and 3 to 6 months specifically in geriatric care. ISNP experience preferred. Additional Opening for a second Geriatric Nurse Practitioner. Geriatric NP/Clinical Supervisor will often act on behalf of physician, performing diagnoses and treatment and interacting with Behavioral, Palliative Care and Transitional Care staff. |
