| Brief Detail: | RN/APRN Care Coordinator to manage RN staff and coordinate a LTC/rehospitalization program. The successful candidate will be responsible for reviewing LTC hospital admissions, reviewing status reports for all residents, evaluating effectiveness of treatment plans, supporting the residents discharge to the community as needed, and providing ongoing care coordination and support to facility staff.
You will be working closely with the nursing, medical and support staff to ensure effective care options to reduce rehospitalizations and coordinate future medical care to achieve this goal. Case Management, Geriatric and Palliative Care experience as well as management experience helpful. This position will include a strong training and education component to support the care coordinators and existing facility based staff and providers. Employer will facilitate regular in-person education sessions to train the RN care coordinators and facility staff on the tools and protocols to use at the facility, in addition to reviewing the challenges in reducing avoidable hospitalizations and readmissions. The training program will be designed to build the care coordinator’s skills to assess facility practices, to coach and support nursing facility staff, and to coordinate with others in the long term and acute care communities.
