| Brief Detail: | We are currently seeking RN's with long term care experience to launce an initiative to reduce avoidable rehospitalizations in the elderly population. You will be working closely with the LTC staff at selected facilities to train them with this new initiative. This position will include a strong training and education component to support the care coordinators and existing facility based staff and providers. There will be regular in-person education sessions to train the RN care coordinators and facility staff on the tools and protocols to use at the facility, training on the use of the required computer programs and reviewing the challenges in reducing avoidable hospitalizations and readmissions. The training program will be designed to build the care coordinator’s skills to assess facility practices, to coach and support nursing facility staff, and to coordinate with others in the long term and acute care communities.
